Modern practice of using non steroidal anti inflammatory drugs to close the arterial duct in premature newborns

Abstract

Hemodynamically significant patent ductus arteriosus is the most common cardiovascular pathology in premature newborns, often determining their survival and further prognosis of full-fledged development. Currently, there is no clearly defined and unambiguously interpreted medical tactics for managing children with patent ductus arteriosus. 190 articles were studied and analyzed in the biomedical research search system Pub Med by the keywords “intravenous ibuprofen ductus arteriosus” and 884 publications in Russian in the Russian scientific electronic library Cyber Leninka by the keywords “open arterial duct”. The analysis of scientific literature was carried out, from which it follows that there are at least 3 drugs from the group of non-steroidal anti-inflammatory drugs that can be used to close the patent ductus arteriosus. For clinical practice, a decision support system is required, based on the principle of ranking patients by gestational age, the degree of opening/non-closure of the arterial duct, and actual hemodynamic and respiratory parameters, in order to select a drug and determine the course dose. A clinical case is presented of successful pharmacological closure of an open arterial duct in a premature infant using acetaminophen and ibuprofen.
